Who We Are

CASA is a division of the Department of Social Services. CASA nurses can provide information on Medicaid programs and care options that may be available to Broome County residents. CASA nurses may complete assessments in the community. Our goal is to help you stay independent as long as you are able, in the least restrictive setting that meets your needs. This may be possible with the help of family, friends, and community resources.

What We Do

CASA nurses may see individuals of all ages, in any Broome County setting.  There is no charge for the CASA nursing visit and assessment.

Effective May 16, 2022, NYS implemented the New York Independent Assessor (NYIA) for all adult initial assessments for Personal Care Aide (PCA) and Consumer Directed Personal Assistance Program (CDPAP) services. Clients must have Medicaid to schedule this assessment.

NYIA is not conducting initial assessments for children under 18 years of age. Please call CASA to discuss these situations.

CASA nurses can explain this new process and what you should expect during and after the initial assessment.


How We Help

Our nurses will help you understand what service options and programs are available in our county. They can explain the Medicaid personal care aide service programs, as well as the Medicaid waiver programs. They may offer recommendations and will answer your questions.

Some of the information that our nurse will discuss with you may include:

MEDICAID PROGRAMS such as:

PERSONAL CARE AIDE (PCA)/SHARED AIDE (SA) SERVICE:  These Medicaid funded programs are for individuals who are chronically ill or are disabled, and who have a medical need for assistance at home. Personal Care Aide service may include assistance with light housekeeping, laundry, grocery shopping and errands and/or hands on personal care tasks such as bathing, grooming, dressing, and personal hygiene.

  • All adult initial referrals must have an assessment by NYIA, contact NYIA at 1-855-222-8350.
  • Initial assessments for children under 18 may be completed by CASA.  To inquire, please call CASA at 607-778-2420.

CONSUMER DIRECTED PERSONAL CARE ASSISTANCE PROGRAM (CDPAP):
This personal care aide service enables the consumer, or a designated representative of the consumer, to direct the delivery of their personal care service and choose (hire) their own worker. The consumer, or their representative, is responsible for all aspects of hiring, training and supervising their personal attendants.

PRIVATE DUTY NURSING:
CASA nurses assess individuals with complex medical needs who require in-home skilled nursing services under the Private Duty Nursing program. These skilled nursing services may be provided by a Registered Nurse or a Licensed Practical Nurse.

NURSING HOME PLACEMENT:
A CASA home assessment may be completed to determine whether an individual is appropriate for Nursing Home Placement. CASA nurses are trained and certified with NYS to complete the Patient Review Instrument (PRI) and Screen that is required for anyone seeking placement in a NYS nursing home. CASA will send this paperwork to the nursing home(s) of your choice. If a county resident does not qualify for nursing home placement, CASA nurses will suggest other options that may meet their needs.

Once a referral is taken, you will be contacted by a CASA nurse to schedule a home visit.
